various Independent Living programs throughout the country (look up JOhn Chaffee for related programs) Foster Youth Services in California Casey family programs http://www.casey.org/ http://www.orangewoodfoundation.org/
I am not an expert on the outcomes research for these nor is this a comprehensive list but hope this helps

Hello All,
I am wondering if anyone knows of programs nationally or internationally that help youth who are aging out of care. If yes, has there been any empirical research conducted on these programs? For example, what kinds of things do these programs emphasize? What kinds of things should these program focus on for youth who are aging out of care.
