Hello Emily, are you familiar with Annie E. Casey Foundations online national statistical data center concerning child well-being? Child fatalities are a data indicator under the Vital Statistics category. https://datacenter.kidscount.org/ This may save you some time from having to contact each state individually. For Arizona, when you bring up the particular data indicator there is a section which reports on the data source with a hyperlink that will take you to that source.
Most state child welfare agencies will publish statements or reports concerning child fatalities/near fatalities. Arizona’s child welfare statements concerning fatalities can be found at: https://dcs.az.gov/news/child-fatalities-near-fatalities-information-releases
Best, of luck.

Subject: Searching for Child Death-Related Investigative Reports 2008-2018
Greetings, colleagues!
I am searching for state (or county-level) investigative reports from which follow the death of a CPS-involved child. These reports usually document the investigation re: the circumstances leading up to the death, the agency’s involvement with the child/family, and also identify “problem areas” and recommendations for change. Just to be clear, these reports are distinct from those issued by child death review teams.
I am doing a systematic review of the content in these reports. In 2015 I collected about 20 such reports, but I am going back and updating the search. I am interested in any reports that were issued between 2008-2018. If anyone can point me in the right direction, I would greatly appreciate it.
Thanks much in advance
Emily Douglas
