Emily, I’m not sure if these fall into the category of child death review teams, perhaps they do, but this link will take you to brief bios of 56 children killed in Minnesota between 2005 and 2013, and embedded in each of them are links to the county review reports (see “Read the mortality review file”). If this is example of what you don’t want then I’m not sure what else would exist, but I’m willing to be enlightened. Rich Gehrman

Greetings!
I am in search of state (or county-level) investigative reports which follow the death of a CPS-involved child. These reports usually document the investigation re: the circumstances leading up to the death and the agency’s involvement with the child/family, and also identify “problem areas” and recommendations for change. Just to be clear, these reports are distinct from those issued by child death review teams.
I have identified 4-5 such reports (MA, FL, VA, CT), but would appreciate learning about more.
